Option 1: North America

I request authorization to make long distance telephone calls over the University's system in the United States, and Puerto Rico.

Option 2: International

I request authorization to make long distance calls in United States, Canada, and International calls.

NOTE - For international dialing privileges you must deposit $100 in your dormitory deposit escrow account. The receipt from Student Accounting should accompany this application when turned in to the telecommunications Office.

I will receive a personal authorization code for which I assume full responsibility. Should this code be lost, stolen or used by unauthorized parties, I will continue to be responsible for all calls until I notify the Telecommunications Office in writing to change my authorization code or terminate my calling privileges.

I understand that all charges will be billed to my student account and I will receive a monthly statement providing a detailed record of long distance calls. I accept responsibility for payment, due upon receipt of my statement, and in the event of late or non-payment, my authorization code will be deactivated until payment is received. A $10 fee will be charged for reactivating of my authorization code. If I change residence or wish to disconnect my long distance dialing privilege, I will notify the telecom office to receive a final statement. I authorize any past due balance to be withdrawn from my student account and.or residence hall deposit.
